1. = link first ocurrence of each glossary item to the glossary definition
2. ? line splitting: patches are smaller, easier to edit in a 80-col term (<ulink>...</ulink>
wants to be on a single line)
3. + use <filename></filename> for filenames

1. Fix width of page headers (in Spanish for example the title "Manual de usuario de
Sylpheed-Claws" doesn't fit and gets badly splitted)
2. Separate page number from horizontal rule at the bottom

1. Fix space introduced after all linked elements (and included in the generated anchor)
